Should you neutralize Milko or let him live in the Phantom Liberty Treating Symptoms gig? Once you reach the basement of the Voodoo Boy’s hideout in Dogtown, the quest and the Netrunner saddle you with a major decision. Though the rewards for completing the quest are no different, it’s a choice worth weighing up beforehand.

To get the option of letting Milko live, make sure you talk to the Netrunner. At the end of the dialogue chain, you’ll get two options:

  • ‘Fine, finish your op.’ Translation: let Milko live.
  • ‘Here to protect my client.’ Translation: kill Milko.

Neutralize Milko – outcome

If you decide to neutralize Milko, the Netrunner turns hostile and attacks you, so you’ll have to deal with him. Loot his body for Alan’s Keycard, Alan Noel’s Dog Tag, and Kanabo melee weapon. Loot Milko to get a Unity pistol

Leave the Voodoo Boy’s base through the pipe at the back of the basement. Once outside, you’ll receive 8000 credits and a bunch of XP for your trouble. A call will come through from Mr. Hands thanking you for completing the job per his instructions.

Let Milko live – outcome

If you choose to let Milko live, the Netrunner says you won’t regret the decision and opens the door at the back of the basement. Leave the base and you’ll get 8000 credits and some XP. 

Mr. Hands will then get in touch to let you know that Indira Barazza, the client, is now on the witness protection list. Netwatch has promised to protect her body and mind and that no hacker can reach her now. 

Additionally, Mr. Hands will be very pleased with the way you handled the job. You guaranteed the safety of his client and elevated his standing with his corporate employers. The main draw of letting Milko live is that neither Milko, the Netrunner, or Indira Barazza die.
