To carry over the Rebirth demo progress to the full game’s release, you simply need to beat the demo and save the game. If you do this, you can skip the first chapter of the full game to save you from playing the same section over from the beginning.

Carrying over demo progress only applies to the Nibelheim flashback section of the game. The second stage of the demo, where you explore Junon, has no impact on the main game, and nothing you do in that save will be carried over.
